In consideration of acceptance of this entry, I hereby waive and release any and all claims against Westside High School, Houston Independent School District and any and all other sponsors and officials of the "Run With The Wolves" Fun Run for any injury or illness which may directly or indirectly result from my participation in this event. I state that I am in proper condition to participate in the event.
